Step 1: Send a Referral
Fax the patient’s face sheet, history, physical, and physician orders to:

Step 2: Verification and Coordination
Our intake team will:
- Verify insurance eligibility and homebound status
- Contact the patient and family to schedule the first visit
- Coordinate with discharge planners and case managers for a smooth transition home

Step 3: Admission and Follow-Up
A Registered Nurse or Therapist will complete an initial assessment within 24–48 hours.
We will send the Plan of Care (CMS-485) to the referring physician for signature and provide regular progress updates.
