Our Process:
- Referral & Intake: Physician or case manager initiates service.
- Assessment: RN evaluates patient’s medical condition and environment.
- Care Plan: Interdisciplinary team creates a personalized care plan.
- Treatment: Skilled clinicians deliver scheduled visits.
- Evaluation: Continuous monitoring and adjustment for optimal recovery.
